Using a Holistic Needs Assessment to guide your CCR (Concerns Checklist)

Did you know, in recent case studies more than 70% of people attending their CCR were not given a Macmillan Concerns Checklist beforehand? (BSW ICB, 2024)

Using a Macmillan Concerns Checklist enables people to prepare for their CCR, think about the concerns they would like to discuss and gives permissive discussion around concerns that they may otherwise feel embarrassed to initiate discussion on. This also enables people to prioritise the discussion points and make the CCR effective in the time given.

The checklist is available in 26 different languages and can be requested directly from Macmillan.

People who have undergone treatment in secondary care will most likely have had an End of Treatment Summary (EOTS) and/or Holistic Needs Assessment (HNA) done. People who have finished their treatment may have their EOTS as a clinical letter (note for clinical coders: please use SNOMED Code SCTID: 1324891000000103). This will list their diagnosis, treatment plan, surveillance (if needed) and GP instruction.

The HNA is an assessment of their concerns, whether physical, practical, emotional, relationships, keeping healthy and signposting needs. You can use this HNA as a baseline when they come for a CCR if the HNA has been sent through from the hospital where they received treatment (note for clinical coders: when a HNA is received – you must code it using the SNOMED code SCTID: 787261000000106).

Both of these documents can be linked in SystmOne to their cancer diagnosis problem and therefore enable a quick assessment of their current situation in preparation for their CCR.

Before attending the CCR, you should send a copy of the Concerns Checklist (primary care version of a HNA) to someone attending their review.  Assess for any adjustments needed and send in either in electronic or paper form, or using the inbuilt AccuRX Florey Questionnaire which collects their concerns, health and smoking status and codes these back to their record in advance of the CCR.
