A CCR gives an opportunity to discuss Advanced Care Planning if someone you are supporting is palliative and approaching end of life.
The appropriate Ardens advanced care planning template can be initiated from the CCR screen and must be used to enter that person onto the End of Life register. Any decisions that are discussed around place of care and RESPECT completion must also be completed on the Integrated Care Record (ICR).
The ICR does not automatically pull the RESPECT form out of SystmOne, instead you must create and publish this on the ICR independently. This can then be seen by ALL health professionals attending that person’s needs. Repeating decisions that have been difficult to arrive at by your patients is distressing and can result in undue conveyance to hospital.
Accessing the ICR is easy, just use the ‘eye’ icon to access the ICR when your patients record is open (shown in bottom left below).
